Greater Cincinnati Chapter AOSA Video Request Form General Information: 1. All recorded programs in our library are either videotapes in VHS format or DVD-R discs playable in most DVD players or computers. All chapter library materials are the property of the Greater Cincinnati Chapter AOSA library. Only Chapter Members with National AOSA membership who pay fees for the entire yearly chapter workshop package or those taking the workshops for credit from the University of Miami are eligible for library privileges.
2. NO PROGRAM MAY BE REPRODUCED FOR ANY REASON!
3. Some VHS videotapes are recorded using a slower tape speed to fit an entire workshop on one tape. This saves mailing costs while only slightly reducing the quality of the video picture. Adjusting the tracking on your VCR may be necessary to get a better picture.
4. The cost for borrowing a workshop program is $8.00, which includes outgoing postage and handling. Checks should be made payable to : Greater Cincinnati AOSA. The borrower is responsible for the return postage cost. We recommend insuring the returning video; borrowers of lost videos will be charged $20 to cover replacement costs.
5. The length of borrowing time is two (2) weeks from the date the video was received. Videotapes/DVDs more than one (1) week overdue will be charged an extra $5 late fee.
